What is it?

It is an extract of birth certificate, issued once in a lifetime, on a special and secured paper, this document is compulsory for the issuance of the biometric passport. Algerian Citizens residing within the 44 States that are under the jurisdiction of The Consulate General of Algeria in New York can apply for the Birth Certificate 12S. The Consulate General of Algeria in New York will issue the Birth Certificate 12S for Algerian citizens born :
    In Algeria through a direct online application on the website, In the United States within the 44 States, through E-mail or mail application, Abroad (besides Algeria) , through mail application.

How to get  your  12S ?

♦ You are born in Algeria , you can get it from :
    The Town Hall of your place of birth ; by proxy (procuration) to someone in Algeria to get it for you, the proxy must be done at the consulate; Online through a website made available to them by the Ministry of the Interior and Local Authorities : http://demande12s.interieur.gov.dz  ( read more how to apply online for 12s).
You are born in United States  : In order to obtain the Birth Certificate 12S, the birth must be already transcribed at either the Algerian Embassy in Washington DC (before November 2011) or at the Consulate General of Algeria in New York.
If the Birth of the child has not been transcribed at the Consulate General of Algeria in New York, please refer to the Birth Registration.
The request can be made by E-mail or mail by providing the followings: Copies of the parents Algerian ID (passport /ID/Consular Card) . Copy of the family book (if you have one). Copy of Algerian Identification (if minor, parents identifications). Copy of the Family Book (if available). 2- If the birth was not transcribed, the applicant must provide: Original Birth Certificate issued by the country of Birth . Application form .
